KINGDOM KEYS MENTORING LIABILITY WAIVER AND RELEASE FORM

In consideration of the participation of my child in The Hangout Summer Program, organized by Kingdom KEYS Mentoring (a 501c3 organization) and any of its affiliates, partners, employees, volunteers, or contractors (collectively, “Kingdom KEYS”), I, the undersigned parent or legal guardian, hereby agree to the following terms:

1. Voluntary Participation: I understand that participation in Kingdom KEYS’ summer camp, including all related activities (such as field trips, transportation, and events), is entirely voluntary and may involve inherent risks.

2. Assumption of Risk: I acknowledge that Kingdom KEYS will take reasonable precautions for safety, but I accept that injuries, accidents, or unforeseen incidents may occur. I assume full responsibility for any risks, whether known or unknown, related to my child’s participation.

3. Release of Liability: I, on behalf of myself and my child, waive, release, and forever discharge Kingdom KEYS, its directors, officers, employees, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage incurred during or in connection with the program.

4. Indemnification: I agree to indemnify and hold harmless Kingdom KEYS from any claims brought by third parties arising from my child’s actions or participation in the program.

5. Medical Treatment: I authorize Kingdom KEYS to obtain necessary emergency medical treatment for my child if I cannot be reached. I accept financial responsibility for any such care.

   
   

6. Acknowledgement: I have read and fully understand this waiver. I voluntarily sign this release, with full knowledge of its legal significance.

KINGDOM KEYS MENTORING

MEDIA RELEASE AND AUTHORIZATION FORM (MINOR PARTICIPANT)

This Media Release and Authorization (“Release”) is executed by the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of the minor participant named below (“Participant”), in favor of Kingdom KEYS Mentoring, Inc., together with its directors, officers, employees, agents, volunteers, contractors, sponsors, partners, and affiliates (collectively, “Kingdom KEYS”).

1. Grant of Permission

For valuable consideration, including the opportunity for the Participant to engage in programs, events, activities, and services provided by Kingdom KEYS, the Parent/Guardian hereby grants to Kingdom KEYS the irrevocable, perpetual, worldwide right and permission to photograph, videotape, film, record, and otherwise capture the Participant’s image, likeness, voice, statements, performances, and/or appearance (collectively, “Media”).

2. Use of Media

Parent/Guardian authorizes Kingdom KEYS to use, reproduce, edit, modify, publish, distribute, display, transmit, broadcast, and otherwise exploit the Media, in whole or in part, in any format or medium now known or hereafter developed, including but not limited to:

• Promotional and marketing materials

• Websites and online platforms

• All social media platforms (including but not limited to Instagram, Facebook, YouTube, TikTok, X/Twitter, LinkedIn, and any future platforms)

• Printed materials, flyers, and advertisements

• Newsletters and publications

• Grant applications and donor communications

• Educational and training materials

• Presentations and public relations materials

• Television, radio, livestreams, and digital broadcasts

  
  

Parent/Guardian understands and agrees that the Media may be posted publicly online and may be shared, reposted, or viewed by the public once published on social media or digital platforms.

3. No Compensation

Parent/Guardian acknowledges and agrees that neither the Participant nor Parent/Guardian will receive any financial compensation, royalties, or other payment for the use of the Media. All rights, title, and interest in and to the Media shall be the sole and exclusive property of Kingdom KEYS.

4. Waiver and Release

Parent/Guardian, on behalf of themselves and the Participant, hereby waives any right to inspect or approve the finished Media or any written or electronic content that may be used in connection with it. Parent/Guardian further releases, discharges, and agrees to hold harmless Kingdom KEYS from any and all claims, demands, or causes of action arising out of or related to the use of the Media, including but not limited to claims for invasion of privacy, defamation, misappropriation of likeness, or any other claim.

5. Indemnification

Parent/Guardian agrees to indemnify, defend, and hold harmless Kingdom KEYS from and against any and all claims, liabilities, damages, costs, or expenses (including reasonable attorney’s fees) arising from or related to any breach of this Release or any claim brought by or on behalf of the Participant.

6. Scope and Duration

This Release shall remain in effect indefinitely and shall apply to all Media captured during the Participant’s involvement in any Kingdom KEYS program, including but not limited to The Hangout Summer Program, workshops, meetings, field trips, transportation, special events, and related activities, unless revoked in writing. Any revocation shall apply prospectively only and shall not affect Media already captured or previously published.

7. Authority to Sign

Parent/Guardian affirms that they are the legal parent or guardian of the Participant and have full legal authority to execute this Release on the Participant’s behalf. Parent/Guardian further affirms that they are at least eighteen (18) years of age and legally competent to sign this document.

8. Governing Law

This Release shall be governed by and construed in accordance with the laws of the State of Florida. Any legal action arising from this Release shall be brought exclusively in the courts located in Miami-Dade County, Florida.
